Please complete OHF's letter and submit it for your VSC
In the "Member" line, please put Ontario Women's Hockey Association
In the "Association/Club" line, please put Huntsville Girls Hockey Association
In the "Age Group and Level" line, please put the division and HL for House League and the division and letter if in Competitive
In the "Team Official/Volunteer Position" line, please put one of the following: Coach, Assistant Coach, Trainer, Manager, Room Monitor or Staff

When you submit your information, you will get an email confirming they received your submission, we call this email your receipt. Save it as a PDF file. You will use it when you upload your VSC.
